When individuals are referred to PAC, a meeting is conducted to assess the most effective method of providing the requested service.
Overseeing this process are a PAC Facilitator, who collects all referral information, and a Community Liaison Nurse, who establishes contact with the individual and their care team to help clarify areas of concern and treatment goals. |
