PAC Referral Process

Admission Criteria


Individuals are admitted to PAC’s in-patient facility voluntarily or involuntarily under the Mental Health Act

Individuals must be dually diagnosed with a developmental disability, psychiatric and/or behavioral issue

Individuals must be 14 years of age or older

A CLBC Facilitator or Analyst must be involved with the individual

All accepted admissions must have a Discharge Plan

Individuals must be medically stable at time of admission

The length of admission is determined by PAC Clinical Team's assessment of the presenting issue

The length of admission may be two weeks to three months

Who Can Refer

An individual can be directly referred to PAC by any of the following:


Caregiver

CLBC Analyst or Facilitator

Mental health support team clinician

Community clinician

Referral Steps


1.

Please contact PAC’s Facilitator Brian Roberts, brian.r.roberts@gov.bc.ca, before any Referral Package sections are filled out.

2.

After establishing eligibility, Facilitator will request the following package to be completed and forwarded to the local CLBC office:

3.

PAC's Clinical Team reviews the referral documents and comes to a decision on an admission. PAC's Facilitator communicates this information to all parties involved.

4.

Facilitator sets up an Integrated Planning Meeting (IPM) between PAC and the individual's care team.

5.

PAC's Community Liaison Supervisor sets a date for a home visit.

6.

PAC's Facilitator communicates assigned admission date to all parties involved.

Admission Procedure


1.

The individual and their primary caregiver arrive at PAC at an agreed upon time. An orientation takes place.

2.

The admission meeting is held.


Our Assessment Process

PAC's Clinical Team develops and implements an assessment and treatment plan with the full involvement of the individual and their support network.
